Becoming a New Patient is as Easy as 1-2-3-4:
1) Click here to download the new patient paperwork "Complete Registration Packet"
2) Submit the completed paperwork via fax, mail, or in-person and verify your insurance benefits and our network status with your plan.
3) Once submitted, a staff member will review it and contact you for scheduling. Please present all insurance information at this time.
4) Come on in for your visit! Please arrive 15 minutes early and bring with you your ID, insurance card(s), and Good Faith Estimate (if applicable).
*NEW: In our effort to reduce waste, we now offer secure online submission of new patient paperwork. This is an alternative method to printing and sending in/dropping off your packet. Just give us a call to get started! Once all paperwork is complete and received, a staff member will contact you for scheduling.
